На базе клиники ФГБНУ «Федеральный исследовательский центр фундаментальной и трансляционной медицины» (г. Новосибирск) обследованы 38 пациентов с артериальной гипертензией и сопутствующим головокружением, проводилось дуплексное сканирование сосудов шеи с помощью ультразвукового сканера Vivid E9.</p><p>Результаты и их обсуждение. Показано влияние состояния кровотока на уровне сонных и позвоночных артерий на выраженность клинической симптоматики головокружения у больных артериальной гипертензией с сочетанным ДППГ. Выявлена клиническая значимость атеросклеротического изменения сонных артерий, степени извитости сонных и позвоночных артерий на эффективность лечения и профилактики головокружения у больных с АГ и ДППГ. Показано, что у больных АГ с ДППГ выраженность взаимосвязи гемодинамически значимых изменений сонных и позвоночных артерий с клиническими проявлениями головокружения больше, чем у больных с АГ без ДППГ. Использование ультразвукового дуплексного сканирования брахиоцефальных артерий у больных АГ, сочетанной с головокружением, позволяет проводить дифференциальную диагностику ДППГ и дисциркуляторных цереброваскулярных нарушений, оценивать степень их участия в клинических проявлениях головокружения, что может служить основой для разработки новой медицинской технологии.</p></abstract><trans-abstract xml:lang="en"><p>Objective of the study was to assess the diagnostic significance of hemodynamic disorders in the area of carotid and vertebral arteries in patients with hypertension combined with benign paroxysmal positional vertigo (DPPG) using ultrasound duplex scanning of neck vessels.</p><sec><title>Material and methods</title><p>Material and methods. Thirty eight patients of the clinic of Federal Research Center for Fundamental and Translational Medicine with hypertension and concomitant dizziness were examined. The state of carotid and vertebral arteries, blood flow in the arteries were assessed by duplex ultrasound scanning using the Vivid E9 apparatus.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. The influence of the blood flow at the level of carotid and vertebral arteries on the severity of clinical symptoms of dizziness in patients with hypertension with concomitant DPPG was shown. The clinical significance of atherosclerotic changes in carotid arteries, tortuosity of carotid and vertebral arteries of the effectiveness of treatment and prevention of dizziness in patients with hypertension and DPPG was revealed. It is shown that in patients with hypertension and comorbid DPP, the severity of the relationship of hemodynamically significant changes in the carotid and vertebral arteries with clinical manifestations of dizziness is greater than in patients with hypertension without DPP. The use of ultrasound duplex scanning of brachiocephalic arteries in hypertension with comorbid vertigo allows for differential diagnosis of benign paroxysmal positional vertigo and cerebrovascular dyscirculatory disorders, the degree of their participation in clinical manifestations of dizziness.
