Это заставляет осуществлять поиск новых предикторов, ассоциированных с неблагоприятными исходами сердечно-сосудистых заболеваний. Особенно большое внимание уделяется изучению влияния экспрессии генов-кандидатов, кодирующих белки – активаторы системы гемостаза при данных патологических состояниях.</p><sec><title>Материал и методы</title><p>Материал и методы. Из 100 участников исследования для проведения клинико-молекулярного анализа, в том числе для оценки экспрессии генов, отобран 61 пациент, которые были разделены на две группы: основная группа, пациенты очень высокого CC риска (n = 43), и группа сравнения, лица без кардиоваскулярных заболеваний в анамнезе (n = 18). У обследованных методом ПЦР проведена оценка относительного уровня экспрессии генов SERPINE1, FGB, ITGA2 и ITGB3, ассоциированных с нарушением гемостаза у пациентов c кардиоваскулярной патологией.</p></sec><sec><title>Результаты</title><p>Результаты. У больных очень высокого СС риска относительный уровень экспрессии генов SERPINE1 и ITGB3 выше, чем у лиц группы сравнения. Содержание мРНК гена ITGB3 существенно больше у пациентов со сниженной фракцией выброса левого желудочка (ФВ ЛЖ) и многососудистым поражением коронарного русла, мРНК гена SERPINE1 – у больных 60 лет и младше, со сниженной ФВ ЛЖ, двусосудистым поражением коронарного русла и стентированием правой коронарной артерии.</p></sec><sec><title>Заключение</title><p>Заключение. Обнаружено значимое повышение уровня относительной экспрессии генов ITGB3 и SERPINE1 по сравнению с фоновой изоформой актина, используемой в качестве контроля нагрузки (ACTB) у пациентов очень высокого CC риска, в том числе с рецидивирующими CC событиями. Уровень экспрессии данных генов и скорость его достижения в группе больных с мультифокальным поражением коронарных артерий и сниженной ФВ ЛЖ, по-видимому, может рассматриваться как универсальный маркер тяжести течения СС заболеваний у пациентов очень высокого CC риска.</p></sec></abstract><trans-abstract xml:lang="en"><p>Individuals at very high cardiovascular (CV) risk are a heterogeneous group, including patients with favorable and unfavorable outcomes, which prompt the search for new predictors associated with adverse CV disease outcomes. Especial attention is paid to studying the effect of expression of candidate genes encoding proteins that activate the hemostasis system in these pathological conditions.</p><sec><title>Material and methods</title><p>Material and methods. Of the 100 study participants, 61 patients were selected for clinical and molecular analysis, including gene expression assessment, and were divided into 2 groups: the main group, patients with very high CV risk (n = 43), and the comparison group, individuals without a history of CV disease (n = 18). In those examined using the PCR method, an assessment was made of the relative expression level of SERPINE1, FGB, ITGA2 and ITGB3 genes associated with impaired hemostasis in patients with CV pathology.</p></sec><sec><title>Results</title><p>Results. In patients with very high CV risk, the relative expression level of the SERPINE1 and ITGB3 genes is higher than in the comparison group. The ITGB3 gene mRNA content is significantly higher in patients with reduced left ventricular ejection fraction (LVEF) and multivessel coronary artery disease, the SERPINE1 gene mRNA – in patients 60 years of age and younger, with reduced LVEF, two-vessel coronary artery disease and right coronary artery stenting.</p></sec><sec><title>Conclusions</title><p>Conclusions. A significant increase in the relative expression level of ITGB3 and SERPINE1 genes was found compared to the background actin isoform used as a load control (ACTB) in patients with very high CV risk, including those with recurrent CV events.
