В настоящее время рутинной практикой является имплантация электрода электрокардиостимулятора в верхушку правого желудочка. Но ряд исследований, выполненных в последние годы, показывает негативные последствия постоянной правожелудочковой стимуляции. Электрическая и механическая диссинхрония, приводящіе к аномальному сокращению желудочков, ведут к ремоделированию миокарда левого желудочка, что в свою очередь вызывает появление и прогрессирование симптомов сердечной недостаточности, появление предсердных аритмий и повышение смертности от сердечно-сосудистых причин. Стимуляция проводящей системы сердца является перспективным направлением в области лечения брадиаритмий, вызывая более физиологический ответ на стимуляцию через проводящую систему Гиса – Пуркинье.</p><sec><title>   Материал и методы</title><p>   Материал и методы. Представлен клинический случай по изменению области постоянной кардиостимуляции у пациентки, имеющей полную атриовентрикулярную блокаду, раннее корригированную электрокардиостимулятором.</p></sec><sec><title>   Результаты</title><p>   Результаты. Пациентке выполнено оперативное вмешательство, заключавшееся в замене электрокардиостимулятора с изменением области желудочковой стимуляции. В результате перехода с апикальной стимуляции на стимуляцию области левой ножки пучка Гиса самочувствие пациентки улучшилось: повысились толерантность к физической нагрузке, качество жизни и изменились некоторые показатели диссинхронии миокарда.</p></sec></abstract><trans-abstract xml:lang="en"><p>   Pacemaker implantation is the primary treatment for bradyarrhythmias such as atrioventricular and sinoatrial block. Currently, it is a routine practice to implant a pacemaker’s electrode into the apex of the right ventricle. However, a number of studies performed in recent years have shown the negative consequences of chronic right ventricular pacing. Electrical and mechanical dissynchrony, leading to abnormal ventricular contraction, leads to remodeling of the left ventricular myocardium, which in turn causes the appearance and progression of heart failure symptoms, the occurrence of atrial arrhythmias, and increased mortality from cardiovascular causes. Pacing of the heart’s conduction system is a promising direction in the treatment of bradyarrhythmias, causing a more physiological response to stimulation through the His-Purkinje system.</p><sec><title>   Material and methods</title><p>   Material and methods. This article presents a clinical case of changing the area of permanent cardiac pacing in a patient with complete atrioventricular block, previously corrected with a pacemaker.</p></sec><sec><title>   Results</title><p>   Results. The patient underwent surgery, which consisted of replacing the pacemaker with a change in the ventricular stimulation area.
